Defect Rate: 100% of affected vehicles

The Problem: The rear view camera harness design and routing can cause damage from movement and vibrations during vehicle operation. Over time, this results in degraded camera display quality, including blurriness, distortion, multi-colored imagery, or complete screen failure.

Safety Risk:

Violation of Federal Motor Vehicle Safety Standard (FMVSS) No. 111 for rear visibility. Reduced rearward visibility increases crash risk, particularly during backing maneuvers.

Warning Signs:

Camera image degradation often occurs gradually, with blurriness or distortion appearing before complete failure. Some drivers may dismiss early symptoms as minor issues.

Parking Pawl Engagement Failure

The Problem: Resistance between the parking rod and wedge inside the transmission housing can prevent proper parking pawl engagement. When the parking pawl fails to engage, vehicles may roll away even when the shifter is placed in “Park.”
Safety Risk: Unexpected vehicle movement after parking creates significant injury and crash risks, especially if drivers don’t engage the parking brake. This is particularly dangerous on inclined surfaces.

Critical:

This defect provides no warning to drivers. The transmission appears to shift into Park normally, but the vehicle may still roll away.

LEAF Sustained Engine Torque

Production Dates: September 29, 2017 – March 15, 2023
Defect Rate: 100% of affected vehicles

The Problem: Faulty Vehicle Control Module (VCM) software can cause sustained engine torque under specific conditions. The issue occurs when drivers perform a specific sequence within 8 seconds after deactivating cruise control systems: switching driving modes (D to B, ECO mode, or e-Pedal ON) and then applying and releasing the accelerator pedal.
Safety Risk: Vehicles may continue accelerating unexpectedly or fail to slow down as expected when the accelerator is released. This unintended acceleration significantly increases crash risk.

No Warning:

This software defect provides no advance warning to drivers and can occur during normal driving operations involving cruise control and drive mode changes.
